Please have your current insurance card available at the time of your visit.

If we participate with your insurance plan, any applicable copayment will be due at check-in.

If we are not in-network with your insurance, if your child is uninsured, or if we do not submit claims to your insurance company, full payment is required at the time of service.

We encourage all families to verify their coverage and benefits directly with their insurance provider prior to scheduling an appointment, so you have a clear understanding of your financial responsibility.
